For quick screening, I accept:
- P411 check
- provider references relevant within one year

Please provide 2-3 of the following choices,

- full name and date of birth
- photocopy of state issued/gov photo ID. You may blur the address line.
- Active and established LinkedIn account
- board handle
- registered phone number (no VoIP accepted)
- provider reference from verifiable and established provider, visit must have been within 24 months

*Selfies, FaceTime and Skype do not qualify for screening.
